Registered Nurse Care Manager - WI

Job Summary

To support patient care and outcomes, the full-time Registered Nurse Care Manager - WI will assess and coordinate care for assigned patients, collaborating with multidisciplinary teams in a hybrid work environment after an initial onsite orientation period.

Key responsibilities

• Reviews patient eligibility for care management and assesses their needs for appropriate care levels
• Coordinates access to services and facilitates patient care progression using best practice interventions
• Documents clinical activities and collaborates with healthcare professionals to eliminate barriers to care delivery

Required qualifications

• Graduate of an accredited school of nursing or equivalent educational background for licensing
• Two years of registered nurse experience
• Registered Professional Nurse (RN) license in the state of Wisconsin or a compact state
• Experience in health coaching, preventative care, or care management preferred
• Familiarity with value-based care and motivational interviewing is a plus
