Registered Nurse Care Specialist
Job Summary
To support members with chronic and complex health conditions, the seasonal, fully remote Senior Care Specialist (Registered Nurse) will conduct telephonic health assessments, develop individualized care plans, and coordinate services to promote improved health outcomes through a Whole Person Care approach.
Key responsibilities
- Conduct telephonic health assessments for Medicare, Medicaid, and group health plan members
- Develop actionable and individualized care plans based on member needs and identified health risks
- Collaborate with multidisciplinary teams to coordinate care and condition-management activities
Required qualifications
- Active, unrestricted Registered Nurse (RN) license in the state of residence
- Minimum of 2 years of clinical nursing experience; 3 or more years preferred
- Strong clinical judgment and critical-thinking skills
- Intermediate to advanced computer proficiency, including Microsoft Word, Excel, and Outlook
- Ability to work independently in a remote environment
