Registered Nurse Case Manager

Job Summary

A company is looking for a Case Manager / RN.

Key Responsibilities

• Manage an individual caseload and coordinate care plans with healthcare providers and payers
• Assess patient needs, set goals, and document case management actions
• Maintain communication with patients and providers to ensure ongoing care and address any non-medical issues affecting patient welfare

Required Qualifications, Training, and Education

• Current active and unrestricted Registered Nurse license in the United States
• Bachelor's degree in a health-related field preferred
• 7 years of varied clinical experience preferred
• URAC-recognized certification in Case Management within 3 years of hire
• Familiarity with community resources and funding sources for patient care
