Registered Nurse Case Manager

Job Summary

A company is looking for a Case Manager (Specialty) / RN.

Key Responsibilities

• Manage individual caseloads and coordinate care plans with healthcare providers, payers, and patients
• Assess patient needs, set goals, and document actions to achieve those goals
• Maintain case management files and ensure timely reporting and billing in the computer system

Required Qualifications

• Registered Nurse (current active and unrestricted license in all 50 states)
• Bachelor's degree in a health-related field preferred
• 7 years of varied clinical experience preferred
• URAC-recognized certification in Case Management within 3 years of hire
• Familiarity with community resources and funding sources for patient care
