Registered Nurse Case Manager

Job Summary

A company is looking for a Case Manager (Acutes) / RN.

Key Responsibilities

• Manage individual caseloads through the case management process, including service authorization and treatment plan reviews
• Assess patient needs and document case management plans, including goals and interventions
• Maintain communication with healthcare providers, payers, and patients to ensure quality care and compliance with benefit constraints

Required Qualifications

• Active and unrestricted Registered Nurse license in all 50 states
• Bachelor's degree in a health-related field preferred
• 7 years of varied clinical experience preferred
• URAC-recognized certification in Case Management within 3 years of hire
• Proficiency in case management software and documentation practices
