Job Summary

A company is looking for a Nurse Case Manager - NICU (CST time zone).

Key Responsibilities

Assess members' needs and develop individualized care plans
Monitor care plans and coordinate with providers to ensure effective service delivery
Provide education to members and facilitate access to community resources

Required Qualifications

Registered Nurse (RN) with a current, unrestricted license
Preferred college degree
Three or more years of clinical experience in a Level III or above NICU
CCM certification required within specified timeframe
Experience with data entry into a database or EHR
