Registered Nurse Case Manager

Job Summary

A company is looking for a Case Manager Registered Nurse - Remote.

Key Responsibilities

• Facilitate the delivery of benefits and healthcare information to determine eligibility and promote wellness
• Develop and implement health strategies, policies, and programs to ensure member wellness and timely return to work
• Utilize a collaborative process for assessment, planning, care coordination, and advocacy to meet comprehensive health needs

Required Qualifications

• Active and unrestricted RN licensure
• Minimum 3+ years of clinical practical experience, preferably with Medicare members
• Minimum 2+ years of experience in case management, discharge planning, or home health care coordination
• Must reside in Central, Mountain, or Pacific time zones
• Associates Degree or Nursing Diploma required; Bachelor's Degree preferred
