Registered Nurse Case Manager

Job Summary

A company is looking for an RN Case Manager to provide member-centric, evidence-based care across multiple products.

Key Responsibilities

• Conduct comprehensive assessments and develop individualized care plans for members
• Coordinate services across the healthcare continuum while collaborating with members, families, caregivers, and providers
• Monitor and implement care plans to reduce illness burden and healthcare costs

Required Qualifications

• Associate's degree in Nursing or equivalent work experience in a related field, plus 5 years of clinical/acute care nursing experience
• Current, unrestricted RN license in the state of residence
• Certified Case Manager (CCM) preferred or commitment to obtain within two years of hire
• Experience with vulnerable and complex populations in clinical, home care, or telephonic environments
• Proficiency in electronic health records and care management software
