Registered Nurse Case Manager

Job Summary

A company is looking for a Case Manager / RN (licensed in all 50 states).

Key Responsibilities

• Manage individual caseloads and communicate with healthcare team members to meet patient needs
• Review medical data and develop case management plans, documenting goals and actions
• Maintain case files, prepare reports, and ensure continuity of care through ongoing patient contact

Required Qualifications

• Current active and unrestricted Registered Nurse license in all 50 states
• Bachelor's degree in a health-related field preferred
• 7 years of varied clinical experience preferred
• URAC-recognized certification in Case Management within 3 years of hire
• Proficiency in using computer systems for case documentation and billing
