Registered Nurse Case Manager

Job Summary

To facilitate the delivery of healthcare benefits and promote wellness activities, the full-time Registered Nurse Case Manager will manage client cases remotely, ensuring eligibility for benefits and supporting health strategies for member wellness.

Key responsibilities

• Facilitate the delivery of healthcare information and benefits to determine eligibility while promoting wellness
• Develop and implement health strategies and programs to support member wellness and timely return to work
• Occasionally travel within a designated area for in-person case management activities as needed

Required qualifications

• Active Compact License or registered KS, MO license
• Minimum 3-5 years of clinical practical experience, preferably with Medicare members in areas such as diabetes and cardiac care
• Minimum 2-3 years of case management, discharge planning, or home health care coordination experience
• Ability to obtain multi-state RN licenses if needed
• Registered Nurse with an active state license in good standing
