Registered Nurse Case Manager

Job Summary

To support a growing healthcare initiative, the full-time Registered Nurse Case Manager will facilitate the delivery of benefits and healthcare information, promote wellness activities, and coordinate care for members in a remote setting.

Key responsibilities

• Manage the assessment, planning, facilitation, and evaluation of care coordination for individuals and families
• Develop and implement health strategies and policies to ensure member wellness and timely return to work
• Collaborate with healthcare providers to determine eligibility for benefits and promote quality, cost-effective outcomes

Required qualifications

• Active and unrestricted RN licensure
• Minimum 3+ years of clinical practical experience, preferably with Medicare members
• At least 2+ years of experience in Case Management, discharge planning, or home health care coordination
• Must reside in Central, Mountain, or Pacific time zones
• Associate's Degree or Nursing Diploma required; Bachelor's Degree preferred
