Job Summary
Providing telephonic case management in a fully remote capacity, the full-time Registered Nurse Case Manager will coordinate resources and develop individualized treatment plans for injured workers in a workers' compensation environment.
Key responsibilities:
- Conduct telephonic outreach for assessments, follow-ups, and care coordination, including formulating clinical case plans
- Maintain regular contact with injured workers, employers, and providers to facilitate communication and monitor treatment progress
- Review case records and medical statuses, ensuring appropriate treatment and timely return-to-work placements
Required qualifications:
- Current, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required; Certified Case Manager (CCM) preferred
- Diploma, Associate, or Bachelor's degree in Nursing or a related field; Master's degree preferred
- Three or more years of clinical experience in acute care and two or more years in medical case management or managed care
- Knowledge of utilization management, quality improvement, and discharge planning
- Proficiency in Microsoft Office applications, including Word, Excel, and PowerPoint
