Job Summary
To facilitate effective healthcare management for CareLink program members, the full-time salaried Registered Nurse Case Manager will coordinate resources, perform assessments, and implement care plans in a 100% remote environment.
Key responsibilities
- Perform essential case management activities while ensuring confidentiality and adherence to legal and regulatory standards
- Utilize motivational interviewing techniques to set measurable goals and coach clients on self-care practices
- Monitor and evaluate the effectiveness of care plans, making necessary adjustments to achieve desired outcomes
Required qualifications
- Nursing degree from an accredited institution
- 2+ years of clinical experience as a Registered Nurse
- Experience in case management or transferable skills
- Active, unrestricted Registered Nurse license with CEUs as required by the State Board of Nursing
- Willingness to obtain Certified Case Manager (CCM) designation within 3 years of hire
