Job Summary
Creating and monitoring treatment plans for patients, the full-time RN Case Manager will assess healthcare needs and coordinate care options while working remotely during standard daytime hours.
Key responsibilities
- Assess members' health status and coordinate care, including inpatient reviews and discharge planning
- Develop and implement short and long-term case management goals with documented care plans
- Monitor health status and treatment plans, adjusting as necessary based on client needs
Required qualifications
- 5+ years of related work experience in clinical nursing and patient assessment
- Bachelor's degree or equivalent related work experience
- Graduate of an accredited school of nursing
- RN - Registered Nurse - State Licensure with Compact State Licensure in good standing
- Knowledge of medical terminology and cost containment practices
