Job Summary
Working remotely, the full-time Registered Nurse Case Manager will conduct comprehensive assessments, develop care plans, and coordinate care for beneficiaries, aiming to enhance their health and functional capabilities through telephonic and digital outreach.
Key responsibilities
- Conduct comprehensive assessments and prioritize clinical, psychosocial, and behavioral concerns for beneficiaries
- Develop and document individualized care plans in collaboration with beneficiaries, families, and healthcare providers
- Monitor and evaluate care plans regularly to ensure effectiveness and make necessary revisions in collaboration with the healthcare team
Required qualifications
- Current, unrestricted Registered Nurse license in the state of residence with the ability to obtain multi-state licensure
- Associate or bachelor's degree in nursing from an accredited institution; bachelor's degree preferred
- 5+ years of clinical RN experience in direct patient care
- Knowledge of case management practices and patient-centered care concepts
- Proficiency in Microsoft Office and mobile technologies
