Registered Nurse Case Manager - NICU
Job Summary
To support optimal wellness for high-risk members, the full-time Remote Registered Nurse Case Manager - NICU, who must be bilingual in Haitian Creole, will assess member needs, develop individualized care plans, and coordinate services across the healthcare continuum.
Key responsibilities
- Conduct comprehensive assessments of members' physical, psychological, and social needs to create tailored care plans
- Monitor and adjust care plans to ensure effective service provision and member progress towards health goals
- Interface with healthcare providers to facilitate care coordination and provide education to members regarding their treatment and services
Required qualifications
- Bilingual in Haitian Creole
- Registered Nurse (RN) with a current, unrestricted compact license
- Three or more years of clinical experience in a Level III or above NICU
- CCM certification required within specified timeframe
- Experience in a Case Management role preferred
