Registered Nurse Chronic Condition Manager

Job Summary

A company is looking for a Chronic Condition Case Manager / RN.

Key Responsibilities

• Manage an individual caseload using Complex and Chronic Condition Management constructs
• Collect and document patient information to formulate and update care plans
• Collaborate with providers, payers, and participants to ensure access to appropriate resources and care

Required Qualifications

• Current unrestricted Registered Nurse license in the state of practice
• Minimum of three years of clinical experience, including at least two years in chronic disease management
• Experience in patient teaching in any healthcare setting preferred
• Proficiency in computer literacy, preferably with Microsoft Office applications
• Fluency in English with strong reading and writing abilities
