Registered Nurse Chronic Disease Coach

Job Summary

Providing personalized guidance and support, the full-time Registered Nurse Chronic Disease Coach will empower members in South Carolina, North Carolina, or Georgia to achieve optimal health and wellness through disease prevention and health promotion, working remotely from Monday to Friday, 8:30 am to 5:00 pm.

Key responsibilities

• Assess member needs related to chronic conditions and develop action plans to support health goals
• Educate members and providers about chronic conditions and promote enrollment in management programs
• Communicate effectively with members and healthcare providers regarding member conditions and care options

Required qualifications

• Associate's degree in a job-related field or equivalent experience
• Three years of professional health and wellness experience
• Working knowledge of Microsoft Office and health coaching principles
• Ability to work independently and prioritize tasks effectively
• Certification in health coaching must be obtained within four years of hire
