Job Summary

Managing complex cardiology caseloads, the full-time Senior Clinical Care Navigator will guide patients from diagnosis through recovery, ensuring seamless care coordination and optimal health outcomes while collaborating with clinical and operational teams.

Key Responsibilities

• Manage a dedicated caseload of cardiology patients and facilitate necessary referrals for testing and procedural appointments
• Participate in the creation and evaluation of educational materials and clinical care paths to ensure adherence to medical plans
• Act as a subject matter expert, providing education on cardiac diagnosis management and enhancing patient experience through coordinated care

Required Qualifications

• Bachelor of Science in Nursing (BSN) from an accredited School of Nursing
• Minimum of 8+ years of progressive clinical experience in Cardiology
• Current and unrestricted Registered Nurse licensure
• Demonstrated ability to lead interdisciplinary teams with minimal supervision
• High level of proficiency in Evidence-Based Practice (EBP) and relationship-based care models
