Job Summary

Managing advanced cardiology caseloads, the full-time Registered Nurse Clinical Care Navigator will guide patients from diagnosis through recovery, ensuring seamless care coordination and operational success while working remotely.

Key Responsibilities

• Manage a dedicated caseload of cardiology patients and facilitate referrals for testing and procedural appointments
• Participate in the creation and evaluation of educational materials and clinical care paths to ensure adherence to medical plans
• Collaborate with management on daily operations to enhance service delivery and patient satisfaction

Required Qualifications

• Bachelor of Science in Nursing (BSN) from an accredited School of Nursing
• Minimum of 5+ years of progressive clinical experience in Cardiology
• Current and unrestricted Registered Nurse licensure
• Proficiency in Evidence-Based Practice (EBP) and relationship-based care models
• Ability to manage complex workflows and advanced patient scenarios
