Job Summary

Managing complex cardiology caseloads, the full-time Senior Clinical Care Navigator will guide patients from diagnosis through recovery, ensuring seamless care coordination and operational success while working flexibly in a remote environment.

Key Responsibilities

• Manage a dedicated caseload of cardiology patients, facilitating referrals for testing and procedural appointments
• Participate in creating and evaluating educational materials and clinical care paths to ensure adherence to medical plans
• Collaborate cross-functionally to support workflow improvements and ensure alignment with patient experience goals

Required Qualifications

• Bachelor of Science in Nursing (BSN) from an accredited School of Nursing is required
• Minimum of 8+ years of progressive clinical experience in cardiology is preferred
• Current and unrestricted Registered Nurse licensure
• Demonstrated ability to lead interdisciplinary teams with minimal supervision
• High level of proficiency in Evidence-Based Practice (EBP) and relationship-based care models
