Registered Nurse Clinical Coordinator

Job Summary

A company is looking for a Clinical Referral Coordinator Nurse for Home Care.

Key Responsibilities:

• Acts as the clinical liaison for patient care referrals, promoting continuity with referral sources
• Handles communication regarding clinically complex referrals with a customer-friendly approach
• Completes and processes referral documentation to facilitate the admissions process and ensures compliance with regulatory requirements

Qualifications:

• Completion of an accredited school of professional nursing is required
• 1 year of experience in home health or hospice care strongly preferred
• Registered Nurse (RN) licensure is required
• Clinical knowledge and critical thinking ability for effective patient care coordination
• Current licensure in the state of practice or a multistate license from the Nursing Licensure Compact (NLC)
