Registered Nurse Clinical Liaison

Job Summary

A company is looking for a Clinical Liaison.

Key Responsibilities

• Manage the immediate referral response process to meet performance goals
• Conduct clinical assessments using electronic medical and client relation management systems
• Evaluate prospective resident referrals based on clinical capabilities, bed availability, and eligibility criteria

Required Qualifications

• Active and in good standing Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice
• Minimum of two years of experience in a similar role within a LTC/SNF/AL/MC setting
• Experience in clinical liaison and admission coordination
• Intermediate to advanced computer skills, including experience with EMR or HER
• Ability to make independent decisions and handle confidential information
