Registered Nurse Clinical Liaison

Job Summary

A company is looking for a Clinical Liaison.

Key Responsibilities

• Manage the immediate referral response process to meet performance goals
• Conduct clinical assessments based on electronic medical and client relation management systems
• Evaluate prospective resident referrals and communicate determinations to referral partners

Required Qualifications, Training, and Education

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice
• Minimum of two years of experience in a similar role within LTC/SNF/AL/MC settings
• Experience in clinical liaison and admission coordination
• Intermediate to advanced computer skills, including EMR or HER experience
• Ability to handle confidential information and make independent decisions
