Job Summary

A company is looking for a Clinical Outreach Specialist, Remote.

Key Responsibilities:

• Conduct targeted patient outreach to close care gaps and ensure timely care transitions
• Engage with high-risk patients for post-discharge follow-up and medication reconciliation
• Collaborate with practices to support care gap initiatives and document interventions accurately

Required Qualifications:

• Bachelor's Degree in Nursing (BSN)
• Active, unrestricted Registered Nurse (RN) license; Valid Compact RN license
• Minimum of 2 years clinical experience in case management or discharge planning
• Experience with telephonic patient outreach and clinical assessments
• Experience with electronic health records (EHRs) and population health tools
