Job Summary

A company is looking for a Registered Nurse Coordinator (Alerts).

Key Responsibilities

• Monitor patient vitals, symptoms, and labs to identify those needing clinical interventions
• Follow up with patients on abnormal readings and triage appropriately
• Lead virtual follow-ups to support program enrollment, treatment plan changes, and medication adherence

Required Qualifications

• 4+ years of experience treating patients with CHF, hypertension, and diabetes
• Compact multi-state licensure (RN compact)
• Experience in a CHF bridge clinic environment
• Experience with remote patient monitoring technology
• Independent thinker with a passion for improving healthcare through digital innovation
