Registered Nurse Disease Management

Job Summary

Working remotely in a full-time capacity, the Telephonic Registered Nurse Disease Management will manage patient education and disease management interventions, perform health coaching for members, and coordinate care plans across multiple chronic disease programs.

Key responsibilities:

• Conduct behavioral or clinical assessments to identify individual member needs
• Develop and implement health coaching plans based on assessment objectives
• Coordinate with healthcare providers to facilitate referrals and support member health goals

Required qualifications:

• Associate's degree in nursing with a minimum of 2 years of relevant clinical experience
• Current unrestricted RN license in applicable state(s)
• Experience in condition-specific clinical or home health/discharge planning
