Registered Nurse Disease Management Coordinator

Job Summary

A company is looking for a Disease Management Coordinator focused on diabetes management.

Key Responsibilities

• Coordinate and evaluate healthcare services to optimize patient health status
• Identify and prioritize patient needs through risk stratification and targeted interventions
• Collaborate with healthcare teams to ensure safe transitions of care and implement individualized care plans

Required Qualifications

• Associate's Degree in Nursing with seven years of clinical experience or Bachelor's Degree in Nursing with five years of clinical experience
• Current Registered Nurse license in the relevant state or multi-state license through the enhanced Nurse Licensure Compact (eNLC)
• Pass a state criminal background check and federal checks as required
• Obtain Basic Life Support certification within 30 days of hire
• Preferred: Bachelor's Degree in Nursing and prior care coordination experience
