Job Summary

A company is looking for a Peak Focused Disease Management Coordinator in Population Health.

Key Responsibilities

• Coordinate and monitor healthcare services to optimize patient health status
• Identify targeted populations and prioritize patient needs for interventions
• Collaborate with healthcare teams to ensure safe transitions of care and individualized care plans

Required Qualifications

• Associate's Degree in Nursing with 7 years of clinical experience or Bachelor's Degree in Nursing with 5 years of clinical experience
• Current Registered Nurse license in the state of service or multi-state license through eNLC
• Completion of a state criminal background check and federal checks as required
• Certification in Basic Life Support within 30 days of hire
• Prior care coordination experience is preferred
