Job Summary

A company is looking for a Registered Nurse Disease Manager for remote positions.

Key Responsibilities

• Perform assessments and provide education and training to coordinate care for dependents of Active Duty and eligible retirees
• Manage disease management programs effectively in a remote setting
• Ensure high-quality patient care and support through remote communication

Required Qualifications, Training, and Education

• Bachelor's Nursing Degree
• At least one active state nurse license, preferably compact state licensure
• Minimum of 2 years' experience as a Registered Nurse Disease Manager, ideally in a remote or hybrid environment
• Preferred certifications in disease-management areas such as Chronic Care Professional (CCP) or Certified Diabetes Educator (CDE)
• Access to secure, high-speed internet
