Registered Nurse Health Coach
Job Summary
A company is looking for a Health Coach Consultant - Registered Nurse.
Key Responsibilities
- Conduct clinical assessments and engage members in chronic condition support and personalized coaching
- Provide individualized telephonic health coaching and support sustainable behavior change
- Develop and maintain individualized care plans while documenting all interactions per regulatory requirements
Required Qualifications
- Registered Nurse with an unrestricted nursing license and ability to be licensed in all non-compact states
- 3+ years of acute adult Medical-Surgical RN experience
- High-speed, hard-wired internet connection
- Proficiency with Microsoft Office and EMR documentation experience
- Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred
