Job Summary

To support patients throughout their healthcare journey, the part-time or full-time Registered Nurse Healthcare Advocate will guide individuals through complex health concerns, develop care plans addressing social determinants of health, and build trusting relationships, all in a remote 1099 contract position.

Key responsibilities

• Guide patients through their healthcare journey, addressing complex health concerns and wellness goals
• Develop comprehensive patient care plans that consider social determinants of health
• Build strong, trusting relationships with patients through empathetic listening and support

Required qualifications

• Registered Nurse (RN) license in good standing
• Experience working with diverse patient populations
• Strong clinical and organizational skills
• Ability to quickly learn and adapt to new systems and technologies
• Demonstrated empathy and a commitment to advocacy for patients
