Job Summary

To enhance patient care coordination, the full-time Registered Nurse - Multi-State Licensed will engage with patients and families, communicate with referral sources, and manage care coordination across departments while primarily working onsite during day shifts.

Key responsibilities

• Facilitate communication with patients and families regarding continuum of care services
• Coordinate care between various departments and manage urgent hospice referrals during weekends
• Conduct eligibility and chart reviews to assess appropriateness for continuum of care services

Required qualifications

• Graduate of an accredited school of nursing or equivalent education for licensing
• Must obtain licenses for MO, IL, OK, and WI within 6 months of starting
• Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association (AHA)
• Experience in Hospice, Home Health, or Case Management preferred
• No prior nursing experience required
