Job Summary

A company is looking for a Nurse Navigator Cardiometabolic.

Key Responsibilities

• Conduct nursing assessments to identify patient needs and collaborate on interdisciplinary care plans
• Provide education and support to patients and families, navigating them through the care continuum
• Coordinate medication access and ensure adherence to treatment regimens while monitoring patient care access points

Required Qualifications, Training, and Education

• Registered Nurse license issued by the state of practice
• Associate's Degree in Nursing
• Typically requires 3 years of experience in cardiology and/or diabetes care
• Certification as a Certified Diabetes Care and Education Specialist (CDCES) and/or Cardiac Vascular Nursing Certification (CV-BC) is beneficial
