Job Summary

A company is looking for a Registered Nurse (RN) for Senior Housing Visits in Passaic, NJ.

Key Responsibilities:

• Conduct routine monthly visits to review resident status and assist in goal progress
• Attend meetings, perform care coordination activities, and communicate updates
• Engage with residents, families, and interdisciplinary care team while assisting with care planning

Required Qualifications:

• Active RN license in the state of practice with no disciplinary actions
• Minimum of 1 year of RN experience
• Experience in Long-Term Care (LTC), care coordination, or case management is a plus
