Job Summary
To support members diagnosed with cancer, the full-time remote Oncology Transitions of Care Nurse will conduct clinical assessments, coordinate care transitions, and educate members and caregivers about their diagnosis and treatment over the phone.
Key responsibilities
- Conduct telephonic assessments and monitor member health progress while providing regular updates and targeted support plans
- Coordinate care transitions, including discharge plans with hospital case managers and follow-up care with providers
- Assist members with care management and address social determinants of health needs, ensuring access to medications and appointments
Required qualifications
- Active, unrestricted compact Registered Nurse (RN) license and willingness to obtain additional state licenses as needed
- Degree in Nursing (ADN or BSN) required; BSN preferred
- At least 5 years of nursing experience with 3 years in oncology nursing or case management, and Certified Case Manager (CCM) certification
- Experience with technology and comfort in learning new tools, including electronic health records and video conferencing
- Fluency in Spanish, both written and verbal, is preferred
