Job Summary
To support a growing oncology care practice, the full-time Oncology Transitions of Care Nurse will conduct clinical assessments, coordinate care transitions, and educate members and caregivers about cancer treatment, all while working remotely or in a hybrid model.
Key responsibilities
- Conduct telephonic assessments and monitor member health changes while coordinating care and discharge plans
- Build strong relationships with members and healthcare providers to optimize care and prevent readmissions
- Assist with care coordination, resource referrals, and address social determinants of health needs for members
Required qualifications
- Active, unrestricted compact Registered Nurse (RN) license and willingness to obtain additional state licenses
- Degree in Nursing (ADN or BSN) required; BSN preferred
- At least 5 years of nursing experience, including 3 years in oncology nursing or case management, and Certified Case Manager (CCM) status
- Proficiency with technology, including electronic health records and video communication tools
- Fluency in Spanish, both written and verbal, is preferred
