Job Summary
To support a growing oncology practice, the full-time Complex Oncology Nurse Navigator will conduct clinical assessments, coordinate care transitions, and educate members and caregivers about cancer treatment, all while working remotely with a schedule from 11:30 AM to 8:00 PM EST.
Key responsibilities
- Conduct telephonic clinical assessments and monitor changes in members' health
- Coordinate care management and discharge plans with hospital case managers and providers
- Build strong relationships with members, caregivers, and healthcare teams to optimize care and address social determinants of health
Required qualifications
- Active, unrestricted compact Registered Nurse (RN) license with a willingness to obtain additional state licenses
- Degree in Nursing (ADN or BSN) required; BSN preferred
- At least 5 years of nursing experience, including 3 years in high-acuity adult oncology
- Certification as an Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM)
- Proficiency with technology and comfort in using electronic health records and communication tools
