Job Summary
To support a growing oncology care practice, the full-time Complex Oncology Nurse Navigator will conduct clinical assessments, coordinate care transitions, and educate members and caregivers about cancer treatment while working remotely or in a hybrid capacity.
Key responsibilities:
- Conduct telephonic assessments and monitor member health changes throughout the oncology care continuum
- Coordinate care with hospital case managers and follow-up providers to ensure comprehensive support for members
- Build strong relationships with members, payers, and providers to optimize care and prevent readmissions
Required qualifications:
- Active, unrestricted compact Registered Nurse (RN) license; willingness to obtain additional state licenses as needed
- Degree in Nursing (ADN or BSN) required; BSN preferred
- At least 5 years of nursing experience, including 3 years in high-acuity adult oncology
- Certification as an Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM)
- Experience with technology and electronic health records, and a willingness to learn new technologies
