Registered Nurse Oncology Navigator

Job Summary

To support a growing oncology practice, the full-time Complex Oncology Nurse Navigator will conduct clinical assessments, coordinate care transitions, and educate members and caregivers about cancer treatment while working remotely or in a hybrid model based in Nashville.

Key responsibilities

• Conduct telephonic clinical assessments and monitor changes in member health
• Coordinate care plans and discharge processes with healthcare teams and providers
• Build strong relationships with members and assist with care management and social determinants of health needs

Required qualifications

• Active, unrestricted compact Registered Nurse (RN) license, with a willingness to obtain additional state licenses
• Degree in Nursing (ADN or BSN) required; BSN preferred
• Minimum of 5 years of nursing experience, including 3 years in high-acuity adult oncology
• Certification as an Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM)
• Proficiency with technology and electronic health records, with a willingness to learn new tools
