Registered Nurse - Palliative Care

Job Summary

To support a growing palliative care team, the full-time Registered Nurse - Palliative Care will provide clinical support, facilitate hospice coordination, and manage patient education remotely while ensuring high-quality care for chronically ill patients.

Key responsibilities

• Triage urgent palliative care needs and provide clinical support to regional palliative physicians
• Facilitate hospice referrals and maintain communication with hospice agencies to ensure continuity of care
• Conduct patient care tasks and engage high-need patients to promote participation in palliative care

Required qualifications

• Graduate of an accredited School of Nursing
• Currently licensed as a Registered Nurse
• 2+ years of experience in palliative-focused care, geriatric home-based care, or hospice care
• Ability to travel to assigned market(s) up to 20%
• Familiarity with Microsoft Office and web-based applications
