Registered Nurse Population Health Coach

Job Summary

A company is looking for a Population Health Nurse Coach.

Key Responsibilities

• Manage a panel of members with complex chronic and behavioral health conditions to improve health outcomes
• Deliver personalized coaching and care planning to support behavior change and self-management
• Coordinate care across providers and advocate for members to ensure seamless healthcare experiences

Required Qualifications and Education

• 3-5+ years of experience in chronic condition management, population health, or health coaching
• Active, unrestricted RN license (multi-state preferred)
• Strong foundation in motivational interviewing and behavior change strategies
• Experience supporting members with both medical and behavioral health conditions is preferred
• Health Coach Certification and/or Certified Diabetes Educator (CDE) is a plus
