Key Responsibilities

• Lead operations and manage a clinical workforce across multiple service provider accounts
• Own performance metrics, ensuring compliance with KPIs and documentation standards
• Oversee staffing, training, and development of clinical teams while maintaining workflow governance

Required Qualifications

• Registered Nurse (RN) with significant clinical experience in high-acuity populations
• Experience managing large clinical teams (25-35 staff) in care management or population health
• Strong operational leadership background in fee-for-service Chronic Care Management
• Demonstrated ability to manage KPI-driven operations with clear targets
• Experience working with EHR systems and care management platforms
