Job Summary

A company is looking for an RN-Referral Liaison, Continuum of Care Remote PRN.

Key Responsibilities

• Coordinate patient referrals and transitions of care
• Collaborate with healthcare providers to ensure continuity of care
• Provide education and support to patients and families regarding care options

Required Qualifications

• Active Registered Nurse (RN) license
• Experience in care coordination or case management
• Knowledge of healthcare systems and referral processes
• Ability to work independently in a remote setting
• Current CPR certification
