Registered Nurse RN - New Jersey Licensed Senior Housing Visits

Job Summary

A company is looking for a Registered Nurse (RN) for a 52-week contract in Passaic County, NJ, to coordinate care for assisted living and Senior Housing residents.

Key Responsibilities:

• Conduct routine monthly visits to review resident status and assist in achieving progress toward goals
• Attend meetings on behalf of the Nurse Practitioner (NP) and communicate updates to the Interdisciplinary Care Team (ICT)
• Perform care coordination activities within the Special Needs Program (SNP) to ensure resident needs are met

Required Qualifications:

• Active RN license in the state of practice, with no open disciplinary actions, probation, or restrictions
• Minimum of 1 year of RN experience
• Long-Term Care (LTC), care coordination, or case management experience is a plus
