Registered Nurse Senior Care Specialist
Job Summary
To support individuals with chronic and complex health conditions, the seasonal, fully remote Registered Nurse Senior Care Specialist will conduct telephonic health assessments, develop individualized care plans, and coordinate services to promote improved health outcomes.
Key responsibilities
- Conduct telephonic health assessments for members and develop actionable care plans based on identified health risks
- Establish health coaching relationships and provide education and support for chronic condition management
- Collaborate with multidisciplinary teams to coordinate care and address barriers to health and well-being
Required qualifications
- Active, unrestricted Registered Nurse (RN) license in the state of residence
- Minimum of 2 years of clinical nursing experience; 3 or more years preferred
- Strong clinical judgment and critical-thinking skills
- Intermediate to advanced computer proficiency, including Microsoft Word, Excel, and Outlook
- Ability to work independently in a remote environment
