Job Summary

Providing telephonic and telehealth care, the part-time Home Health Telehealth Registered Nurse will manage triage assessments, case management, and patient coordination remotely on weekends from 8 AM to 5 PM EST or CST.

Key responsibilities

• Conduct triage assessments and manage incoming patient/caregiver calls during assigned shifts
• Educate and follow up with patients and caregivers on chronic disease management and coordinate care with healthcare providers
• Document all patient interactions and participate in interdisciplinary team meetings to ensure optimal patient care

Required qualifications

• Current RN license in the state of practice
• 2-3 years of clinical practice experience in a relevant field
• 1-3 years of home health or hospice experience
• Knowledge of disease management and chronic disease processes
• Excellent telephone communication skills
