Job Summary

Registered Nurse Telephonic Care Manager is a full-time remote position responsible for care coordination and health education for Health Plan members through telephonic collaboration, focusing on care transitions and comprehensive care plans.

Key Responsibilities

• Conduct comprehensive assessments and develop individualized care plans in collaboration with members and providers
• Coordinate care and facilitate communication among members, caregivers, and healthcare professionals
• Document all activities in the care management tracking system and identify opportunities for improvement

Required Qualifications

• Minimum of 2 years of experience in a clinical setting and case management nursing required; BSN preferred
• Registered Nurse (RN) licensure required
• Case management certification or approved clinical certification preferred
• Ability to interact professionally with healthcare professionals
• Private, secure designated workspace required in the home office setting
