Required Qualifications

• Registered Nurse with an unrestricted nursing license and multi-state/compact privileges
• 3+ years of clinical/nursing practice experience
• 1+ year of medical case management, care coordination, or discharge planning experience
• High-speed internet access with the ability to hard wire a computer
• 3+ years of experience with personal computers and MS Office Suite applications
