Registered Nurse Transplant Case Manager

Job Summary

A company is looking for a Transplant Case Manager, RN.

Key Responsibilities

• Manage and coordinate care for transplant cases and complex disease cases
• Perform clinical assessments and utilize clinical guidelines to facilitate medical service coordination
• Document case management processes and ensure compliance with HIPAA and regulatory standards

Required Qualifications

• Graduate of an accredited school of nursing
• Minimum of one year experience in Utilization Management or Case Management
• At least three years of transplant experience
• Intermediate knowledge of Microsoft Office applications
• Must maintain an active Registered Nurse license and be eligible for Certified Case Manager certification
